A retroperitoneal puzzler
Clinical history: An 18-year-old woman
presented to her physician with a complaint of abdominal pain. A subsequent CAT scan
revealed a mass, initially thought to be a pedunculated leiomyoma. Exploratory surgery
demonstrated a 12 x 6.5 x 5.5 cm mass involving the small bowel mesentery.
Pathology: A well-circumscribed ovoid mass was present within the
mesentery but did not involved the wall of the attached small bowel segment. Sectioning of
the mass demonstrated a purple-tan parenchyma with small punctate foci of necrosis, as
well as a large necrotic zone centrally. Scrape preparations were made (below left, 100x,
below right, 400x).
